Please supply the information about me I am entitled to under the Data Protection Act 1998 relating to: [give specific details of the information you want, for example

• your personnel file;
• emails between ‘A’ and ‘B’ (between 1/6/11 and 1/9/11);
• your medical records (between 2006 & 2009) held by Dr ‘C’ at ‘D’ hospital;
• CCTV camera situated at (‘E’ location) on 23/5/12 between 11am and 5pm;
• copies of statements (between 2006 & 2009) held in account number xxxxx).

Remember........ the 40 days does not start until payment (if requested, a maximim of £10) is made.

I've used it recently few times and I have some documents back.
Most of the time they will ask for £10 fee.

I never ask for specific information.
I always 'require' any and all information held in all formats.

Ok let anyone know if one knows how to get around this one, ok the UK care act 2014 in it section on adult safe guarding in statute law UK,
Even one never consented to a referral made,
IE, if one has illness disability, physical mental illness disability, subject data access request is blocked, claims in data protection act 1998 , section on this , , not that clear till one read ,both section in care act 2014, on data sharing, in line with adult safe guarding, what I paid for twice over since July 2017 to-date 25.20.2017
The public body data controller claims able to denie my data claim on other third parties data protect against. Yet I never asked for another man woman data only my own , it's lot of reading to do, know how decision are made which may have serious effect on data sharing abuse of incorrect information used stored used ,more so housing companies,police NHS, anyone working public funding company or private companies,in relation to statute law in line with both,care act 2014/safe guarding claim duty,UK

I wonder if one ever come across this before in data access request?
