Potential legal action letter from mortimer clark solicitor

hi there
i need some help please, i had a debt with argos credit card for £313.04, now they sold it to cabot and cabot passed on to there solicitor mortimer clark.
i have followed three letter process since March 2017 with estopell and bills to mortimer clark solicitor who is acting for cabot, today after few months i received a letter from mortimer clark solicitor threatening for potential legal action along with income expenditure form and other form same as county court claim form and i have to reply to them within 30 days from today.
can someone please point me to the right direction and what is my next step please, i already sent them a bill last year for £1000 pound for not paying in advance, (invalid contact by mail without paying agreed fee in advance of £100)

So is that an actual court claim, with claim number etc?  Some DCAs send out what they call a specimen or example form as a sort of threat, or to con people into thinking it's gone to court.  If unsure, you can ring the court.  If it's real you need to act on it.

It’s not real one it doesn’t have any claim number court logo or anything, this form made by them.
I have uploaded the letters and forms bellow.

that is a letter before action, which they must send out to you before issuing a claim

you need to respond within 30 days to stop them issuing a claim, you can request documents as per box I, this is keeping with the new pre action protocol. They should not issue a claim if you have requested documents until they have provided them.

use box I and put in it see separate letter

then write a short letter requesting documents that they will rely on in any claim, and also send an official CCA request

send it all together with the forms and they will back off and try and find some sort of documents to fulfil your request.

I have been nosing around on this site tying to find the CCA template letter but I can't seem to locate it.
Has anyone got a copy they could paste please.
