to get your Form of Information go to the Office of birth deaths & marriages - 29 mins in the previous post of DI:

The form information I think would be your certificate of live birth or live borne record – when born to get a new NHS number the certificate of live birth or live borne record is created.

Has anyone received their "Form of Information"?

Have been here https://www.gov.uk/general-register-office
then here https://www.gro.gov.uk/gro/content/certificates/login.asp where payment is needed
the email could be certificate.services@gro.gov.uk

Question is, what do we ask for?
Can assume, i require the Form of Information for the person 1st name, 2nd name, surname and DoB
Anyone know if this is right?
